Job Description:
Seeking an experienced RN Case Manager to provide hospital-based case management, including discharge planning, utilization review, care coordination, and patient assessment. Must have strong clinical experience and familiarity with hospital workflows. Epic and InterQual experience preferred.

Requirements:

  • BSN
  • Active CA RN License
  • AHA BLS
  • 2+ years clinical hospital RN experience
  • 2+ years hospital Case Management experience
  • Discharge Planning & Utilization Review experience
  • Epic experience preferred
  • InterQual knowledge preferred
  • No first-time travelers

Schedule: Monday–Friday | 8:00 AM–4:30 PM | 40 hrs/week 

RN Case Manager – M–F 8AM–4:30PM | 40 hrs/week | No Call | 2+ yrs Hospital Case Management & UR/Discharge Planning | BSN, CA RN & AHA BLS required | Epic/InterQual preferred.
