Job description

Chart Pre-Loader

Coos County Family Health Services (CCFHS), a Federally Qualified Health Center (FQHC), is seeking a detail-oriented and highly organized Chart Pre-Loader to support provider and clinical support workflows and patient care coordination. This position is responsible for pre-loading critical clinical information into patient charts in AthenaOne, prior to appointments. The ideal candidate will have prior experience using AthenaOne and possess strong attention to detail, confidentiality, and communication skills. This position may be performed remotely anywhere in the United States, in a hybrid setting, or in person depending on organizational needs and candidate qualifications.

Essential Duties and Responsibilities

  • Review daily provider schedules and make necessary adjustments to patient charts in AthenaOne prior to appointments, ensuring patient information, including medications, allergies, diagnoses, family history, social history, and surgical/procedure history, is accurately reconciled and migrated from the previous EMR system.
  • Conduct pre-visit outreach to patients scheduled for Medicare Annual Wellness Visits to gather required health information, complete screening questionnaires, and ensure documentation meets Medicare requirements.
  • Identify missing or incomplete documentation and coordinate with appropriate departments to obtain needed information.
  • Verify preventive care gaps, health maintenance items, and documentation readiness prior to appointments.

Documentation & Compliance

  • Maintain confidentiality and comply with HIPAA regulations at all times.
  • Follow organizational workflows, policies, and FQHC documentation standards.
  • Maintain accurate electronic records and data entry.

Additional Responsibilities

  • Collaborate with providers, nursing staff, front office staff, and medical records personnel, as needed.
  • Participate in staff meetings and training as required.
  • Perform other administrative or clinical support duties, as appropriate and assigned.

Qualifications Required Qualifications

  • High school diploma or equivalent required.
  • Minimum of 1 year of experience using AthenaOne required.
  • Experience working in a medical office, healthcare setting, FQHC, or health information environment preferred.
  • Strong computer skills and ability to navigate electronic medical records efficiently.
  • Excellent organizational skills and attention to detail.
  • Ability to work independently and manage multiple priorities.
  • Strong written and verbal communication skills.
  • Knowledge of HIPAA and patient confidentiality standards.

Preferred Qualifications

  • Experience with preventive care workflows, patient outreach, and wellness screening processes preferred.
  • Medical terminology knowledge preferred.
