Job description

CNA In Long-Term Care Facility

Job Title: CNA

Profession: Nurse Aide

Specialty: LNA

Duration: 17 weeks

Shift: Day and Evening

Hours per Shift: 8, 16

Experience: Minimum of 1 year in LTC/SNF

License: Must be licensed in New Hampshire

Certifications: BLS certification required

Must-Have: Prior LTC/SNF experience preferred

Description: Department: Long-Term Care Facility Type: 100 Resident Bed Capacity; 96 Semi-private Beds, 4 Private Beds Facility Structure: 2 Floors (1 Long-Term Care and 1 Short-Term Nursing Facility) Patient Monitoring: Dementia and Alzheimer's patients wear a tracker that locks the door if they get close Charting System: Electronic Medical Record and paper charting Staffing Ratios: 1 to 9-12 Common Diagnoses: Diabetes, Chronic Obstructive Pulmonary Disease, Post-Stroke, Heart Disease, Fractures, Dementia, Alzheimer's, Rehabilitation, Chronic Conditions, and Multiple Comorbidities

Job Duties: Assist with activities of daily living, including bathing, toileting every 2 hours, peri care, assist with meals and feeding, take patients to activities, assist residents with appointments, bedtime routines, and conduct rounding every 1-2 hours

Experience Requirement: Minimum 1 year, prefer 2 years; first-time travelers accepted, but experienced travelers preferred

Procedures: Occasional IV antibiotics; potential for PICC line management

Monitoring: No telemonitoring required

Staff Support: Supervisor available on every shift as a resource

Physician Availability: Nurse Practitioner rounds each morning; two physicians available during evenings and twice a month

Interdisciplinary Team: Physical Therapy, Occupational Therapy, Speech-Language Pathology, Case Managers, Social Workers, and Dieticians

Respiratory Treatment: Managed by LPNs and RNs, no respiratory therapists on-site; equipment includes CPAP, nasal cannula, and high-flow

Medication Management: Bubble packs for patient prescription medications; house stock of generic medications available

Shift Structure: Day shifts from 7 AM to 3 PM or 7 AM to 7 PM; Evening shifts from 3 PM to 11 PM; Night shifts from 11 PM to 7 AM

Weekend Requirement: Every other weekend

Holiday Requirement: May be asked to work holidays

Floating Requirement: May float between floors as needed

Dress Code: Any professional-looking scrubs; restrictions on jewelry and hair

Department Specific Orientation: 3 shifts with a preceptor required
