This remote role is designed for professionals who thrive in a virtual work environment. You will utilize secure teletherapy platforms to connect with clients, ensuring their privacy and maintaining the highest standards of care. Regular supervision and opportunities for professional development will be provided to enhance your clinical skills in our hospice settings.

• Conduct thorough psychosocial assessments to identify clients' strengths and areas of need in their hospice journey.
• Deliver evidence-based therapy and counseling services tailored to individuals, families, and groups, utilizing techniques such as CBT and DBT.
• Work closely with healthcare providers to develop comprehensive, integrated care plans that support clients’ overall wellness, including their emotional, physical, and spiritual needs.
• Advocate for clients by connecting them with community resources, support services, and advocacy groups ensuring their rights are upheld.
• Maintain detailed documentation of client interactions, treatment progress, and patient outcomes using our electronic health record system.
• Engage actively in team meetings, case conferences, and continuing education opportunities to improve practice and client care approaches.

You will support a diverse clientele, including terminally ill patients and their families navigating the complexities of hospice care. Your role is crucial in providing emotional support and guidance, helping clients deal with grief, loss, and end-of-life decisions. You will become an integral part of their care team, ensuring they feel understood and valued during one of life’s most challenging times.

• Master's degree in social work from an accredited program.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.
• Minimum of two years of post-licensure experience in clinical social work services, ideally in hospice or palliative care settings.
• Proficient in conducting diagnostic assessments and developing tailored treatment plans based on client's unique needs.
• Exceptional interpersonal and communication skills, with a strong ability to build rapport with clients from diverse backgrounds.
• A deep commitment to cultural competence, social justice, and ethical practice throughout all interactions.

• Experience with grief counseling and bereavement support.
• Familiarity with hospice care policies and procedures.
• Proficiency in telehealth practices and technology.

In recognition of your valuable contributions, we offer a competitive salary ranging from USD 115,000 to 130,000 annually, depending on your experience and caseload. Our benefits include:
