• Facilitate educational and therapeutic group sessions to support patients in processing their experiences and emotions. This includes documenting individual patient goals, progress, and reactions during group activities.
• Actively participate in treatment-planning meetings, providing insights and feedback on patient status and objectives. Document updates and modifications to treatment plans based on patient progress and challenges.
• Utilize advanced conflict resolution strategies to address resistance or conflict within family dynamics or among the healthcare team, assisting clients in reshaping unproductive behaviors and thought processes.
• Maintain accurate and up-to-date patient records, employing approved documentation formats to ensure compliance with social work standards. This encompasses all assessments, treatment interventions, family engagement efforts, and case closure processes.
• Develop personalized treatment plans that highlight strengths, address psychosocial risks, and outline actionable steps to meet the unique needs of each patient.

You will work closely with patients who require palliative and hospice care, focusing on emotional, psychological, and social needs. Our patient population includes individuals confronting terminal illnesses, as well as their family members, who also need support navigating the complex emotional landscape of end-of-life care.

• Master’s degree in Social Work (MSW) from an accredited institution.
• Active licensure as a Clinical Social Worker (LCSW) in the state of California.
• Exceptional verbal and written communication skills tailored for engaging with patients, families, and interdisciplinary team members.
• Proven ability to establish and maintain professional boundaries while building trusting relationships with clients.
• Strong time management skills with a commitment to timely documentation and service provision.

• Previous experience in a hospice or palliative care setting is highly desirable.
• Familiarity with electronic health record systems to streamline documentation workflows.
• Demonstrated expertise in crisis intervention and handling sensitive family dynamics.

The compensation package for the Clinical Social Worker Specialist role ranges from USD 120,000 to 130,000 annually, with salary commensurate to experience and caseload.
