• Conduct thorough clinical assessments to understand the needs and challenges faced by clients, developing personalized treatment plans that reflect their individual circumstances.
• Implement evidence-based interventions, such as cognitive behavioral therapy and solution-focused therapy, tailored to promote the well-being and resilience of clients.
• Maintain accurate clinical records, ensuring that all documentation complies with HIPAA guidelines and our internal privacy policies.
• Collaborate with a multi-disciplinary treatment team, including psychiatrists and nursing staff, to provide holistic care and coordinate services for client support.
• Provide education and support to patients and their families, empowering them to understand mental health challenges and engage effectively in the treatment process.
• Stay abreast of developments in the field of mental health, continually enhancing your knowledge to provide the best possible care to clients.

In this role, you will support a diverse client population that includes individuals dealing with grief, loss, chronic illness, and mental health challenges. Working within a hospice setting, you will assist clients in processing their experiences and emotions, playing a crucial role in their overall mental health as they navigate complex life transitions. Your compassionate approach will help create a safe space for clients to express their thoughts and feelings openly.

• A Master’s degree in Social Work from an accredited institution is required.
• Must possess a valid LCSW license to practice in the state of Tennessee.
• A minimum of 2 years of clinical experience is essential, demonstrating proficiency in working with diverse populations.
• Familiarity with solution-focused psychotherapy interventions and trauma-informed care practices is highly valued.
• Exceptional communication and interpersonal skills, allowing you to effectively engage with clients and collaborate with team members.

While not mandatory, experience within hospice or palliative care settings may be beneficial, as well as training in advanced psychotherapy techniques. Experience using electronic health record (EHR) systems to document client interactions and treatment planning may also enhance the efficiency of your work.
