NORTH & CENTRAL ILLINOIS DIVISION
PATHWAY FORWARD PROGRAM
POSITION DESCRIPTION
POSITION TITLE: Reentry Case Manager
LOCATION/DEPT: Pathway Forward Program
REPORTS TO: Program Manager - Case Management
FLSA CATEGORY: Non-Exempt

PAY RANGE: $21.81 - $26.17
OUTCOMES
Carries an assigned caseload of residents currently in the custody of the Federal Bureau of Prisons (B.O.P.) or the United States Probation and Pretrial Office. Provides comprehensive case management services targeted to enhance resident competencies related to a successful transition into the community. Works with each assigned resident to promote a productive lifestyle, reduce the risk of recidivism, and address specific factors that impact the individual resident's criminality and reentry. Oversee the residents' progress through the re-entry program from intake to release. Ensures that all case files are properly organized, up to date, and accurate. Contributes to promoting an overall rehabilitative environment at the re-entry center.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Intake
  • Works with the Intake Department and all other Pathway Forward Program staff to ensure that resident files are properly set up and all necessary intake and orientation procedures are completed on time.
  • As part of the intake process, undertakes a comprehensive assessment of the resident's strengths, needs, and risk factors for recidivism.
  • Upon residents' completion of required intake/orientation, forwards request for Program Review Team (P.R.T.) initial staffing. Prepares 'Program Staffing Report' in advance of initial staffing and presents case at P.R.T. staffing.

Resident Interaction
  • Meets with each assigned resident every 14 days, entering comprehensive "Supervision Plan" at each meeting which reflects the resident's current accountability and re-entry progress including their progress in achieving goals established in the 'Preliminary Treatment Plan'.
  • Maintains assigned case files that are complete, accurate, and current. Responds in timely manner to audits
  • Ensures responsiveness to resident concerns.
  • Enters required resident movements into the community according to program guidelines/parameters. Monitors resident movement in the community and ensure resident accountability.
  • Communicates with the Learning Resource Center (L.R.C.) to ensure resident completion of requirements. Assists the resident identifying employment, school, or job training opportunities appropriate to the resident's education and skill set.
  • Maintains collaborative relationships with outside agencies. Facilitates referrals of residents to outside job readiness/job placement agencies, job training/certification programs, or school programs.
  • Conducts weekly telephonic spot checks after resident starts employment to ensure resident's continued job attendance.
  • Assist the residents to be compliant with income reporting. Ensure verification of first pay date and pay cycle via conferral with the resident's employer.
  • Assist the residents with formulating a budget based on verified monthly income and expenses.
  • Conduct visits to medical facilities to complete furlough documentation as needed.
  • Addresses all special requests, unusual circumstances. Must also address certain emergencies immediately, as they arise. Send special reports to B.O.P. for such occurrences.
  • Monitors GPS tracking and monitoring requirement to ensure that residents are not deviating from their designated sites. When a malfunction occurs with a GPS unit, RCM refers the residents to the Intake Department for troubleshooting or replacement of the GPS units.
  • Reviews residents Home Confinement (H.C.) Eligibility and prepares the resident for "Home Confinement (H.C.)" placement.
  • Processes residents' requests for out-of-district relocation.
  • Ensure that residents abide by the rules and regulations of the Pathway Forward Program.
  • Issue "Disciplinary Incident Reports" and Informal Resolutions for residents not compliant with such rules and regulations.
  • Prepares the residents for release from custody.
  • Completes a comprehensive Terminal Report which includes a summary of the resident's program participation and the services he/she received. The Terminal Report also outlines the date of completion and outcome of such items.
  • Meets individually with Program Manager - Case Management for staffing of assigned cases.

  • All other duties as assigned.

REPORTING RELATIONSHIPS
  • This position reports to the Program Manager - Case Management.
  • In contacts related to this position's duties, this individual acts as a representative of The Salvation Army and its mission.

PERFORMANCE MEASUREMENTS
This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy, and completeness of performance.
A performance review will be conducted.
EDUCATION/EXPERIENCE
  • A bachelor's degree in social/human services, criminal justice, behavioral science, or relevant discipline.
  • Previous relevant work experience can supplement/substitute for educational requirements.
  • Previous case management and/or counseling experience is strongly preferred.

COMPETENCIES
  • Personal maturity and a demonstrated ability to be firm and compassionate. Also, an ability to build effective attitude and behavior changing relationships with residents who differ widely in terms of their personality, education, employment skills, ethnic background and socioeconomic status.
  • Excellent organizational skills as well as aptitude in verbal and written communications.
  • Excellent communication and interpersonal skills, using diplomacy and good judgment.
  • Demonstrated ability to use initiative and be a self-starter.
  • An attention to detail.
  • A positive attitude and the ability to be flexible in light of changing job situations/priorities.
  • Has integrity; a strong commitment to the Salvation Army's vision of human service and social justice, and a clear personal desire to assist in accomplishing the Salvation Army's mission of rehabilitation, restorative justice, and human healing.

POSITION LIMITATIONS
  • This individual will only commit Army resources that have been allocated or approved.
  • This individual will keep the Program Manager - Case Management informed on all critical issues relating to their area of responsibility.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.

PHYSICAL DEMANDS/WORK ENVIRONMENT
  • This position is required to do light physical work and must be able to lift 15 pounds.
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC.
  • The work environment for this position is a residential environment with a low to moderate noise level.
  • Maintain a positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors. This would include but not be limited to such actions: resolution of conflicts in a positive manner; courteous treatment of staff, clients, and visitors; respect of others' property and person, professional and appropriate communication to and about co-workers, supervisors, and subordinates.
