Job description

  • DESCRIPTION OF POSITION
    • Responsible and accountable for managing and directing of day to day operations of Case Management. Works cooperatively with leadership of other departments to integrate clinical services, administrative and educational aspects throughout the continuum. Has 24 hour accountability.  Responsible for planning, development, implementation and evaluation of the Case Management department for Morris Hospital & Healthcare Centers; provides leadership and guidance for functions related to Case Management, Utilization Review, and Social Services.  Develops strategies to manage high risk patients in our community and reduce re-admissions and progression of care delays.
  • QUALIFICATIONS
    • Professional/Educational
      • Bachelor's Degree in Nursing  with Current IL RN Licensure or Master's Degree in Social Work required. 
      • Five years of healthcare or business experience, at least three of which have been in a supervisor or manager.
      • Two years previous experience in case management/utilization review.
      • Certification in Case Management preferred.
      • Experience in RAC auditing / review.
      • DCFS Mandated Reporter Certificate of Completion required.

      • Demonstrated ability to manage both clinicians and non-clinicians.
      • Proven leadership and interpersonal skills
      • Ability to exhibit / promote progressive management skills in order to facilitate change.
      • Ability to envision the whole picture while focusing on major issues.
      • Demonstrated flexibility and dedication to team involvement.
      • Capable of accepting new challenges and development of programs from start to finish.
      • Self-directed with sound decision making and communication skills.
      • Ability to promote team building skills and to empower others.
      • Ability to give both oral and written presentations to physicians and staff.
