Case Management Intensive Care Coordination Supervisor

Annual Salary: $57,119

Work Schedule: Monday - Friday 8:30 a.m. - 5:00 p.m., with on-call responsibilities

The Case Management Supervisor (Intensive Care Coordination) provides day-to-day oversight of intensive care coordination and case management services for children, adolescents, adults, and families impacted by serious emotional disturbance, mental illness, intellectual/developmental disabilities, and substance use disorders. This position ensures services are timely, compliant, and clinically appropriate while strengthening staff performance and program results.
Key responsibilities include supervising approximately 6โ€“8 Case Managers; guiding clinical assessments, service planning, utilization management, and crisis support; coordinating hospital liaison and discharge planning; assigning and balancing team caseloads; training and onboarding staff; partnering with community stakeholders (including courts, law enforcement, and health providers); monitoring unit expenditures; and recommending program/policy improvements.
Required qualifications/skills:
- Bachelorโ€™s degree in Human Services such as Social Work, Psychology, Sociology, or Counseling.
- Three (3) years of experience providing case management services to the assigned client population.
