Job description

Title: Student Case Management Assistant

Position Type: Student Hourly

Job Summary:

The Student Case Management Assistant is a part-time, temporary, grant-funded role designed for currently enrolled students who are interested in supporting student wellbeing and success through case management and behavioral health support. The position will collaborate across the Office of Student Conduct & Support (Care Team only), the Counseling Center (non-clinical mental health case management), and the Student &
Community Resource Center in cases involving mental health, behavioral wellness and basic needs.
Each semester, the Care Team supports an average of 200+ students through individualized case management. Student concerns range widely, from academic support and stress management to anxiety, depression, interpersonal relationships, and suicidal ideation. This role provides valuable hands-on experience in helping manage those cases by connecting students to appropriate campus and community resources.

Duties & Responsibilities:

Care and Mental Health Case Support
• Assist with managing student outreach and referrals for mental health, behavioral concerns, and basic needs.
• Support students in identifying short-term, long-term, and emergency needs, and refer them to appropriate campus and community resources.
• Collaborate with Case Managers to support trauma-informed, inclusive outreach strategies.
• Participate in Care Team meetings and assist in assessing levels of concern or risk.
• Input and maintain accurate case notes and communication logs in the Maxient database.
• Assist with follow-up tasks, documentation, and tracking student progress.
Administrative & Program Support
• Assist in coordinating case schedules and documentation across departments.
• Help manage internal communications among departments regarding student support plans.
• Participate in development and delivery of resource materials and workshops related to wellness, care referrals, and behavioral health.
• Support marketing and outreach efforts for mental health and wellness services.
• Assist in compiling assessment and evaluation data related to case outcomes and student support services.
Cross-Departmental Collaboration
• Provide general assistance to the Counseling Center, including front-desk or scheduling support when needed.
• Coordinate with the Student & Community Resource Center to support holistic wraparound services for students.
• Serve as a student representative on departmental committees or working groups as appropriate.

Competencies/KSAs:

• Strong interpersonal and communication skills, with sensitivity to diverse student needs.
• Interest in supporting student wellness, mental health, or higher education services.
• Ability to manage confidential information with discretion and professionalism.
• Willingness to learn case management tools and software (training provided).

• Coursework or career interest in Human Services, Counseling, Social Work, Psychology, or related areas.
• Familiarity with campus mental health and wellness resources.
• Experience working with or within underrepresented communities.
• Bilingual (Spanish/English) a plus but not required.

Minimum Qualifications:

• Must be currently enrolled at CCD with at least 6 credit hours.
