Job description

The Case Management Assistant responsibilities include:
  • Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
  • Responsible for assisting with distributing regulatory documents to identified patients
  • Educating patients about the appeal process related to their discharge
  • Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
  • Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives

Position details:
  • Location: Morton Plant Hospital - Clearwater, Fl
  • Status: Part time; 16 hours per week
  • Schedule: Saturday & Sunday 8:00 AM - 4:30 PM
  • Weekend Requirement: Occasional
  • On Call: No

Education:
  • Required High School or Equivalent
  • Preferred Associate's

Experience:
  • Required 1 year in Medical Related Field OR
  • Required 1 year Customer Service skills
