Job description

Springpoint Choice is seeking an experienced Full-Time Care Navigator to join our team!
Springpoint Choice, a program of Springpoint Senior Living, offers older adults a membership-based plan to age in place. It combines care coordination, home support, and long-term care coverage, helping members remain independent in their own homes. This position is based out of the Springpoint Choice office at The Moorings at Lewes, located in Lewes, DE.
Responsibilities:
  • Schedule and complete pre-admission assessments of prospective Members which may include memory testing, depression screen, nutritional and home assessment and functional guidelines (ADLs/IADLs) on an as needed basis.
  • Manage a caseload of Members, including regular communication with Members and when necessary families or extended families, assessments of care giving needs, coordinating and scheduling in home services and facilitating admission to facility-based services (provided by the Plan).
  • Initiate and activate the Member's Service Plan and monitor the ongoing provision of care rendered by the credentialed vendor as needed. Maintain detailed records of all Member activity in the Member's file and ALINE system.
  • Provide guidance and support to members or Medical Power of Attorney (POA) with activating long term care insurance policies and participate in long-term care insurance evaluations when possible.
  • Review and approve vendor billing records prior to submitting to accounts payable for Member caseload each month as required.
  • Maintain up-to-date files on resources available in the community for seniors.
  • Assist Member in initiating Member account with Springpoint Choice contracted concierge services provider included in Springpoint Choice membership.
  • Represent Springpoint Choice at professional meetings, networking opportunities, community organization meetings, etc.
  • Participates in monthly Care Navigation calls with all Springpoint Choice Care Navigators.
  • Participate in the on-call rotation.
  • Assist Member with access to Member's local Life Plan Community LivWell program included with Springpoint Choice membership.
Education and Experience:
  • LSW or LCSW preferred
  • Minimum of 3 years' experience working with seniors in a healthcare and/or long-term care environment preferred
  • Experience in case management or related field with senior population
