Intake Coordinator (RN or Therapist)

Position Details: 

  • Full-Time Night Shift - (3)-12 hr shifts from 6:00pm - 6:30am with rotating weekend schedule.

Your goals and objectives include:

  • Responsible for facilitating direct admissions from Medical Hospitals.
  • Perform initial exam of patient, nursing assessment, treatment plan activities for all in-coming and potential patients.
  • Consulting with potential patients, patient families, medical staff and other hospital staff, and
  • Counseling patients and patient families with regard to current assessments, recommended referrals and recommended required levels of care. 

Qualifications

Your Qualifications and Requirements will include:

  • Master’s degree in social work or mental health counseling OR a BS Nursing (BSN) REQUIRED
  • Unrestricted and valid Utah license REQUIRED
  • Minimum 2 years' of post-graduate related experience in psychiatric or substance abuse treatment REQUIRED
  • Experience in settings that include inpatient or partial hospitalization preferred
  • Familiarity with managed health care process, medical terminology, experience in case management, discharge planning, and/or utilization review preferred.
  • Must be at least 21 years of age or older and able to pass a thorough background check and drug screen
