Position Summary:

The Licensed Clinical Social Worker for Case Management assists patients who present with complex, psychosocial needs and work collaboratively with the Case Management Team, Health Care Providers, and Physicians. Intervenes to help the patient and/or family understand and cope with personal, emotional and biopsychosocial issues affecting hospitalization and the delivery of care. Interventions include comprehensive assessment and referral, crisis intervention, and care management. The social worker collaborates with a multi-disciplinary team and community agencies to facilitate a safe discharge plan through the continuum of care.

POSITION QUALIFICATIONS:

Education:

Required: Master's Degree in Social Work

Required: Licensed Clinical Social Work License

Experience:

Prefer one-year supervised experience in a medical facility (hospital, clinic, rehabilitation center, etc.) where a team approach isutilized. Preference shown to individuals with clinical experience.

Training:

Basic knowledge of personality, psychodynamic and crisis theories, techniques of brief counseling/crisis intervention, community resources, and how to access them.

License/Registration/Certification:

Current American Heart Association BCLS certification

Current LCSW License
