Position Summary:

  • Directs all daily patient referral and intake operations including implementation of intakeย  policies.
  • Ensures compliance with all state, federal, and Joint Commission referral/intakeย  regulatory requirements.
  • Screens all referred patients to meet home health eligibility criteria or admission criteria. Directs the implementation of improved work methods and procedures to ensureย  patients are admitted in accordance with policy.
  • Establishes and maintains positive working relationships with current and potentialย  referral sources.
  • Ensures seamless transition of patients to home care by providing patient education andย  preparation for home care, plan of care initiation, and coordination of care with multipleย  service providers.
  • Ensures maximum third party reimbursement through insurance verification andย  authorization processes.
  • Plans & organizes work assignments and sets priorities. Identifies opportunities forย  additional or improved services to meet customer needs.
  • Maintains comprehensive working knowledge of ANX Home Healthcare contractualย  relationships and ensures that patients are admitted according to contract provisions.
  • Maintains comprehensive working knowledge of community resources and assistsย  referral sources in accessing community resources should services not be provided byย  ANX Home Healthcare. Redirects or refers patients to other resources when agency isย  unable to meet care needs.
  • Communicates and coordinates with the home health staff in providing the mostย  accurate information to initiate first home health visit.
  • Behaves in a courteous, professional manner with patients, staff and health careย  professionals and maintains patient confidentiality.
  • Participates in Departmental Quality Assurance Activities and Utilization Reviewย  activities as requested.
  • Participates in in-service programs. Selects and attends education programs to keepย  current in area of practice.ย 

Position Qualifications:

  • Minimum experience of one (1) year as a home health LVN
  • Has current professional license as an LVN in California.
  • Ability to access electronic databases to obtain or enter pertinent patient information.ย 
  • Ability to work in a fast-paced environment with multiple interruptions.
  • Knowledge of governmental regulations, Joint Commission standards, and privateย  payer practices.
  • Demonstrates good written and verbal communication skills. Demonstrates good negotiation and public relations skills.
