What You'll Do

As a Home Health LVN, you'll provide skilled nursing care under the direction of an RN Case Manager while helping patients safely recover and remain independent in their homes.

Your responsibilities will include:

  • Deliver skilled nursing visits in accordance with the physician's orders and individualized plan of care.

  • Monitor patient conditions and promptly communicate changes to the RN Case Manager and interdisciplinary team.

  • Provide wound care, medication management, treatments, and patient education.

  • Reinforce disease management and self-care techniques with patients and caregivers.

  • Collaborate closely with RN Case Managers to ensure high-quality, coordinated care.

  • Document patient assessments, interventions, medications, and progress accurately and timely.

  • Promote patient safety while helping prevent avoidable hospitalizations and emergency department visits.

  • Build trusting relationships with patients, families, physicians, and care team members.

  • Maintain compliance with agency policies, HIPAA regulations, and clinical standards.

What You'll Need
  • Graduate of an accredited Practical Nursing program

  • Current, unrestricted LVN license

  • Minimum of one (1) year of nursing experience in a clinical setting

  • Home Health or Hospice experience preferred, but not required

  • Ability to travel throughout the assigned territory

  • Reliable transportation, valid driver's license, and current auto insurance

  • Strong communication, organizational, and time management skills
