Job description

A skilled nursing and rehabilitation facility in the Clifton Springs area is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our care team. The ideal candidate will be reliable, professional, and committed to providing excellent care to residents in a supportive long-term care environment.


Rates are based on experience.

We also offer a $5000 sign on bonus!

Duties:
โ€ข Collect and record resident health information, including vital signs and medical details
โ€ข Administer medications and injections as prescribed by providers
โ€ข Provide wound care and assist with dressing changes and other treatments
โ€ข Supervise Certified Nursing Assistants (CNAs) as needed
โ€ข Support residents with personal care and daily living activities
โ€ข Monitor and document residentsโ€™ nutritional and fluid intake/output
โ€ข Collaborate with the nursing team to maintain the highest standards of care

Requirements:
โ€ข Valid New York State LPN license in good standing
โ€ข Strong teamwork and communication skills
โ€ข Compassionate approach to resident care
โ€ข Prior long-term care experience preferred
