Job description

Seventh Haven Care Services is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide comprehensive clinical care to clients in a home care setting. This role includes conducting health assessments, assisting with care planning, and performing skilled nursing procedures to ensure each clientโ€™s medical needs are safely and effectively managed in their home.

  • Conduct initial and ongoing client health assessments
  • Assist in the development and implementation of individualized care plans
  • Provide skilled nursing services, including medication administration, wound care, and treatments as ordered
  • Monitor client health status and communicate concerns to the care team
  • Support and follow established care plans to ensure continuity of care
  • Maintain accurate and timely clinical documentation in compliance with state and agency standards
  • Educate clients and caregivers on health conditions, medications, and care techniques
  • Ensure compliance with Minnesota DHS regulations, as applicable
  • Collaborate with physicians, RNs, case managers, and other team members

Qualifications

For Registered Nurse (RN):

  • Current, active RN license in Minnesota
  • Associate or bachelorโ€™s degree in nursing (2โ€“4-year degree required)

For Licensed Practical Nurse (LPN):

  • Current, active LPN license in Minnesota
