Home Insteadยฎโ€ฏis looking for a energetic, dynamic, competitive and sales-oriented LPN to join our team as a Case Manager. If you have community home care experience, and are ready to use your skills in a real leadership role by guiding families through the "Home Care Maze", this position might be for you. You will be relied upon by our clients and their families to help navigate the retirement and care options they are facing. The LPN Case Manager is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/Care Pro introductions and quality assurance visits with existing clients. They use our unique consultative sales approach to determine each individual clientโ€™s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. The LPN Case Manager will often speak with families who are unsure of which pathway is the โ€œbestโ€ so our LPN Case Manager must have a confident, outgoing personality with the ability to communicate professionally & effectively.

This leadership position will include teaching and training our unregulated workforce. This will include classroom training, as well as in home visits to train and support our amazing Care Professionals.

The LPN Home Care Case Manager will work with families who are seeking guidance regarding care options for their elderly loved ones. In this role you will use genuine empathy as well as advanced spoken and written communication skills s to guide potential clients through the confusing maze of choices and care options. The LPN Case Manager role gives you the freedom to do much more, when you meet with families to assess their needs, develop collaborative care plans and continuously work with them to "navigate their care journey". This is much more fulfilling then simply leasing a room and getting a sale, this is about improving the life of our clients. The LPN Case Manager is responsible for effectively managing multiple client demands that come with their changing needs.โ€ฏ This individual must be able to work in a fast-paced organization and have excellent communication skills, be service-oriented and can work well in a team environment. Knowledge of health care will help you understand the unique challenges our clients face. You will be busy and we are looking for that special person who loves hard work! This is how we do Home Care. No 2 days will look alike as you will be visiting clients and their families throughout the city!ย 

To be successful in this role, knowledge of home and community care is required. This role is very different than a general practice LPN Role or facility based LPN role. LPN's who have experience are welcomed to apply if they also have community home care experience.

The Ideal Candidate must:

  • Have a valid LPN license and registrationย 
  • Minimum 2 years of experience with community-based Home Careย 
  • Min 3 years of sales experience
  • Have advanced verbal and written English communication skills (NO EXCEPTIONS)
  • Be a confident communicator
  • Have teaching/training experience
  • Supervisory experience is an asset
  • Be comfortable with a very fast paced, results driven work environment
  • Have a valid min class-5 driver's license
  • Have a clear criminal background and Vulnerable Sector check
