Wound Care Nurse / Infection Preventionist (LPN) - Skilled Nursing

Tutera Senior Living & Health Care is seeking a Wound Care Nurse / Infection Preventionist (LPN) to join our clinical team. This position is ideal for a Licensed Practical Nurse (LPN) with experience in wound care, skin assessments, infection prevention, long-term care, skilled nursing, assisted living, memory care, or rehabilitation.

The Wound Care Nurse / Infection Preventionist is responsible for supporting the facility’s skin management program, providing wound care oversight, and helping lead the facility’s infection prevention and control program. This role works closely with nursing leadership, direct care staff, physicians, nurse practitioners, therapy, and other interdisciplinary team members to promote quality resident care and regulatory compliance.

Responsibilities

As the Wound Care Nurse / Infection Preventionist, you will:

  • Oversee and support the facility’s wound care and skin management program
  • Assess wounds, pressure injuries, surgical sites, ulcers, feeding tube sites, abscesses, and other skin concerns
  • Develop, implement, and monitor wound treatment plans in accordance with physician orders and facility policy
  • Complete skin assessments and monitor residents for changes in condition
  • Provide wound treatments and ensure treatments are completed as ordered
  • Monitor for signs and symptoms of infection and ensure appropriate follow-up
  • Track and report wound progress, healing status, and changes in condition
  • Communicate with physicians, nurse practitioners, families, nursing staff, and the interdisciplinary team as needed
  • Assist with infection prevention surveillance, audits, tracking, and reporting
  • Support infection control practices, including hand hygiene, isolation precautions, personal protective equipment use, and outbreak prevention
  • Provide education and coaching to nursing staff regarding wound care, skin prevention, infection control, and documentation
  • Assist with regulatory compliance related to wound care, infection prevention, and resident care standards
Qualifications

Qualified candidates should have:

  • Active Licensed Practical Nurse (LPN) license in good standing
  • Prior wound care experience required
  • Prior experience in a skilled nursing facility, long-term care facility, assisted living, memory care, or rehabilitation setting preferred
  • Experience with infection prevention, infection control, or infection preventionist duties preferred
  • Strong assessment, documentation, and communication skills
  • Ability to identify changes in condition and follow up appropriately
  • Professional appearance, dependable attendance, and a resident-centered approach to care
  • Ability to educate, coach, and support nursing staff
