We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our evening shift team at Greendale Park Nursing & Rehab, a premier nursing and rehabilitation center located in Greendale, Wisconsin. As a Licensed Practical Nurse (LPN) on our team, you will have the opportunity to provide high-quality care to our residents while working collaboratively with our interdisciplinary team of healthcare professionals.

Responsibilities:

  • Provide direct patient care to our residents, including administering medications, wound care, and treatments as prescribed by physicians

  • Develop and implement individualized care plans to meet the unique needs of each resident

  • Monitor and report changes in resident condition to the registered nurse and/or physician

  • Maintain accurate and up-to-date records of resident care and treatment
