Phoenix Home Care and Hospice is hiring an LPN Clinical Manager to lead and support our Home Health team in Independence. If you are the nurse everyone already turns to for answers, support, and problem solving, this is your opportunity to take the next step.

This role combines clinical leadership, field support, and care coordination in a fast paced home health environment where your experience and decision making truly matter.

Support and guide field nurses and caregivers
Assist with care plans and clinical oversight
