Key Responsibilities

• Communicate with physicians and other healthcare professionals to report on patient progress, addressing any needs and concerns that may arise during treatment.
• Maintain accurate clinical records, including timely clinical and progress notes as per departmental policies, ensuring compliance with healthcare regulations.
• Collaborate with the healthcare team to develop individualized treatment plans that reflect the psychosocial assessments, leveraging recognized social work interventions.
• Provide counseling and crisis intervention to patients, conducting assessments and addressing emotional and behavioral challenges effectively.
• Assist clients with discharge planning, facilitating connections with various service providers in the region to ensure access to necessary resources post-treatment.
• Coordinate closely with families and support systems to ensure comprehensive understanding and planning for client treatment and discharge.

As an LPC at Pathways Wellness Center, you will work with a diverse patient population, which includes individuals facing various mental health challenges such as anxiety, depression, trauma, and substance abuse issues. Qualifications

• A Master’s degree in Professional Counseling from an accredited institution is required.
