• Perform comprehensive assessments to evaluate the needs and mental health status of clients.
• Engage with supervisors, direct care staff, family members, and other stakeholders to discuss client progress and care plans.
• Create and maintain a detailed schedule of appointments, home visits, and observations, ensuring clear communication regarding the treatment pathway for each client.
• Conduct home visits as required, which may involve observations, chart reviews, and staff interviews to provide well-rounded support for each client's needs.
• Implement effective treatment plans and monitor progress, ensuring strategies align with each client's health goals.
• Participate in Interdisciplinary Team Meetings to coordinate care plans and share important insights about client development.
• Document client progress through monthly and quarterly notes to maintain comprehensive records that are accessible to relevant parties.
• Facilitate training sessions to enhance team members’ competencies in therapy methodologies and documentation standards.
• Provide ongoing support to clients through follow-up interviews and assessments to address any evolving needs.
• Act as a liaison within the mental health community, supporting initiatives aimed at improving awareness and access to mental health resources.
• Assist in developing training materials and procedures to ensure ongoing program improvement and staff development.

The LPC will focus on a varied client population, including both adults and youth, addressing various mental health needs. This role offers the chance to work with individuals facing challenges such as trauma, substance misuse, and familial issues. Understanding the diversity of client experiences will be crucial in tailoring therapeutic interventions that are effective and beneficial.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.
• A minimum of two years of practical experience in counseling, with preference given to applicants with a clinical background.
• Specialization in family therapy, couples therapy, substance misuse, or trauma is highly desirable.
• Strong knowledge of applicable legal and ethical standards necessary for mental health practices.
• Exceptional interpersonal and communication skills, with the ability to build rapport and trust with a wide range of clients.
• Demonstrated ability to work collaboratively within a healthcare team.
• Proficiency in Electronic Medical Records (EMR) software and related technologies.

• Experience in assessing and treating complex cases, particularly in outpatient settings.
• Background in designing and implementing program plans focused on recovery and holistic health.
• Membership in relevant professional organizations may be advantageous.
