Job description

Shift: 10A-6P, Monday- Friday in our Discharge Lounge, 32 hours/week

Responsible for overseeing daily operations of the Discharge Lounge and facilitating safe, timely, and efficient patient discharges to the lounge. This role partners closely with inpatient nursing units, providers, care management, transportation services, and ancillary departments to improve patient flow, reduce discharge delays, and enhance the patient experience. The coordinator assesses patient readiness for discharge, provides ongoing monitoring and support while patients await transportation arrival. This position plays a key role in supporting organizational throughput goals, optimizing bed capacity, and ensuring a smooth transition.

  • Coordinate and manage patient flow through the Discharge Lounge.
  • Assess patients for appropriateness and eligibility for discharge lounge services.
  • Collaborate with nursing, physicians, care management, pharmacy, transportation, and support services to eliminate discharge barriers.
  • Promote patient satisfaction by providing a welcoming, comfortable, and supportive lounge experience.
  • Track and communicate discharge lounge metrics, patient throughput data, and operational challenges.
  • Support organizational initiatives related to capacity management, length of stay reduction, and patient flow optimization.
  • Maintain compliance with regulatory, safety, infection prevention, and documentation standards.

  • Must be 16 years old or older
  • Excellent verbal and written communication skills with the ability to interact effectively with patients and interdisciplinary team members.
  • Self-starter with a strong customer service focus, excellent organizational skills, and proficiency with computer systems and applications.
