About the Role
Longevity Health is looking for a Nurse Practitioner or Physicians Assistant with a passion for working with seniors in long-term care settings. In this role, you'll be responsible for a panel of enrolled members receiving nursing home care. You'll be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members. As part of your role, you'll conduct comprehensive assessments and develop individualized care plans for each member. You'll actively participate in nursing facility care team meetings, maintain frequent communication with members' families and loved ones, and ensure the care team stays aligned with members' goals and needs.
Key Tasks and Responsibilities:
  • Provides collaborative primary care and services to a panel of members
  • Delivers ISNP Model of Care activities, develops and implements advanced care planning and embraces/executes a treat in plan model to avoid unnecessary hospitalizations
  • Facilitates and administers Health Plan clinical operations functions-prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promotes membership growth
  • Serves as a trusted advisor to members, loved ones and nursing home staff and colleagues
  • Delivers effective and frequent communication and coordination with members' PCPs, loved ones and interdisciplinary care team members on members' care needs, goals of care, treatment plans/changes
  • Supports overall market goals/targets and balances member caseloads
  • Promotes Longevity Health with members, loved ones and nursing facilities and other stakeholders
  • Is able to function autonomously and is accountable for practice
  • Establishes meaningful relationships and is able to effectively function in the nursing home environment
  • Is member-centric and customer service oriented, establishing meaningful relationships that ensure effective and high-quality care for Longevity Health members
  • Performs other related duties as assigned
Requirements
Knowledge, Experience and Education:
  • 6+ months experience as a Nurse Practitioner or Physicians Assistant
  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master's Degree from a nationally accredited program.
  • Experience working in the nursing home environment is strongly desired
  • Computer/Technology proficiency required
  • The ability to engage and communicate effectively with physicians, members, and family members
  • Self-directed but functions well in a team environment
  • Excellent clinical skills-assessment, diagnosis, treatment, prescribing specifically for the chronically ill population
  • Demonstrated critical thinking skills, organization, and prioritization skills
Credentials & Coverage:
  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner OR state unrestricted licensed Physician's Assistant NCCPA board certified required
  • DEA License required.
  • Must have an active national provider number (NPI) and an active Medicare provider number.
  • Must be eligible to participate in the Medicare program as a provider and registered.
  • Valid state driver's license with a good driving record and proof of automobile insurance required.
  • Auto liability insurance declaration page with minimum limits required by your state of residence. You will be responsible for providing this proof of coverage on an annual basis.
