Job description

Description

Case Management, a program of SWYFS, will provide systems navigation and trauma ย intervention activities for young people of color who have experienced racial profiling, racial bias, and either personally experienced or witnessed prejudice and bias in the legal system, maltreatment, and police brutality toward friends, family, and community members of color.ย 

The Case Manager will establish supportive, consistent relationships with young people, enroll them in case management, and create Individualized Service Plans (ISPs) with goals that are informed by the voices and choices of the young people served.ย  ISPs help young people clearly identify their short and long-term goals, the steps they need to take to accomplish those goals, and the barriers they need to address to be successful.ย ย  ย 

The focus population for this position is people of Black/African American/African Descent.ย 


Essential Functions:

Program Implementation

  • Develop/implement appropriate goals and objectives using Individual Service Plansย 
  • Maintain an active caseload of 15-20 participants each, follow-up with them regularly, provide support and resources, and monitor their progress towards ISP goals. ย 
  • Meet with participants weekly and conduct formal ISP reviews with participants every 90 days.ย 
  • Removes barriers and empowers participants to take control of their lives through building self-confidence, identifying and achieving personal goals, and connecting participants to available resources. ย 
  • Advocate for young adult and work with community/educational/support service agencies to overcome access barriersย 
  • Support young people to develop their voice and advocate for themselves in mainstream systems.ย 
  • Provide in-depth systems navigation including education, immigration, legal, public benefits enrollment, housing, utilities, employment systems navigation, and basic and emergency assistance.ย 
  • Coordinate and facilitate support group as needed, including Aggression Replacement Trainingย 
  • Collaborate with other case managers as neededย 
  • Maintain client confidentiality.ย 
  • Use standardized forms for intake and assessment to enroll young adultย 
  • Provide age and culturally competent servicesย 
  • Assist with tracking and monitoring timelines to meet Young Adult Initiative reporting deadlinesย 
  • Coordinate and collaborate with network-identified agencies, as necessary, to ensure youth are actively engaged in programsย 
  • Develop and maintain working relationships with participating programs to ensure successful referrals and communicating with direct serviceย 
  • Attend community meetings, case management meetings, and other meetings as neededย 
  • Maintain cooperative working relationships with educational, social, legal, and health service agenciesย 
  • Maintain contact with youth to identify additional needs or barriers to service access; to provide support and encouragement; and to follow up to ensure connection with program.ย ย 

ย Documentation and Performanceย 

  • Maintain compliance with documentation with the agency's client database, ensuring all interactions are logged within 24 hours. ย 
  • Meet regularly with the Case Management team to coordinate services provided.ย 
  • Attend the monthly City of Seattle meetingย 
  • Attend weekly case consultation and/or team meetings.ย 
  • Perform other duties as assigned by the Case Management Program Manager.ย 

Requirements

Minimum Qualificationsย 

  • Experience working effectively with diverse populationsย 
  • Experience providing referral, advocacy & case management servicesย 
  • Valid WA State Driver's license, proof of insurance and reliable transportationย 
  • Clear driving record with no more than two moving violations in the last three yearsย 
  • BA Preferred or equivalent work experienceย 
  • Computer skills requiredย 
  • Must be willing to complete a Background Check and Fingerprintingย ย 
  • Demonstrated ability to communicate in writing and verballyย 
  • Demonstrated ability providing community outreachย 
  • Demonstrated ability to utilize time management techniques to organize and prioritizeย ย 
  • Ability to be flexible and adapt to programming changeย 
  • Work evenings & weekends as neededย
