Help Patients Start Their Home Care Journey with Confidence

Are you a compassionate Registered Nurse who thrives in a fast-paced, collaborative environment? Do you enjoy coordinating care and making a meaningful impact from the very beginning of a patient's home health experience?

Revival Home Health Care is seeking an experienced Intake Registered Nurse (RN) to join our growing team. In this hybrid role, you'll work alongside hospital staff, physicians, and care teams to ensure patients transition safely and seamlessly from the hospital to home.

What You'll Do

Coordinate and assess home health care needs for patients referred to the agency.

Serve as the liaison between patients, families, physicians, hospital staff, and community providers.

Complete all intake documentation accurately and within established timeframes.

Develop individualized Plans of Treatment based on physician orders, patient needs, insurance guidelines, and agency protocols.

Collaborate with physicians, nurses, case managers, and discharge planners to coordinate appropriate services.

Obtain initial insurance authorizations and communicate with managed care organizations.

Arrange durable medical equipment (DME), medical supplies, and ancillary services.

Educate patients and families regarding home health services and available community resources.

Serve as a knowledgeable resource for internal and external stakeholders.

Demonstrate a strong understanding of Medicare, Medicaid, and third-party payer reimbursement.

Maintain patient confidentiality while delivering culturally sensitive care.
