Medical Director (Nurse Practitioner (NP)) โ€” Rochester, NY (Monroe County)

Location: Rochester, NY (Clinic-based, in-person)

Schedule: Full-time

Compensation: $130,000โ€“$160,000 base salary, plus eligibility for bonus, comprehensive benefits including medical, dental, vision, 401(k), PTO, and $3,000 CME

Each patient is supported by a dedicated Medical Assistant โ€“ a concierge partner who builds longโ€‘term trust, eases caregiver burden, and leverages AIโ€‘enabled workflows to improve chronic disease outcomes.

The Opportunity

Town Square Health is hiring a Medical Director (Nurse Practitioner (NP)) in the Rochester area to serve as both a clinical leader and a practicing provider in our Medicare-focused, value-based care environment. This is a unique opportunity to help launch and grow our first market โ€” combining handsโ€‘on patient care with meaningful leadership responsibilities.

As Medical Director, you will lead our provider team, serve as a key liaison to central clinical leadership, and play a pivotal role in building the relationships and clinical culture that will define Town Square Health's presence in the Rochester community. You'll also help develop our provider pipeline by identifying and engaging clinical talent in the local market.

This is a clinicโ€‘based role with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20โ€“40 minute visit lengths based on patient needs, and a teamโ€‘based care model with two Medical Assistants paired to each provider.

Leadership Responsibilities
  • Serve as the clinical leader for the Rochester center, holding overall accountability for care quality, provider performance, and clinical culture
  • Directly supervise and support staff providers, serving as their primary point of escalation and professional development resource
  • Act as the key connector between the Rochester clinic and Town Square Health's central clinical leadership team
  • Lead the growth of our first market โ€” building and nurturing relationships with local health systems, specialists, community organizations, and referral partners
  • Partner with operations and recruiting to build a strong local provider pipeline, actively engaging clinical talent in the Rochester market
  • Contribute to clinical protocol development and continuous quality improvement initiatives
  • Leverage data and analytics to drive clinical quality, including Stars and HEDIS measure performance and accurate risk documentation
Clinical Responsibilities
  • Deliver highโ€‘quality primary care to a Medicare patient panel in a clinicโ€‘based setting
  • Coordinate care across the team, including Medical Assistants and specialist colleagues
  • Help shape and continuously improve a care model designed for better outcomes and patient experience
  • Maintain excellent documentation and compliance standards
  • Direct patient care 3.5 days per week (28 hrs)
Qualifications
  • Nurse Practitioner (NP), DNP or equivalent experience strongly preferred
  • National certification in at least one of the following specialties:
    • Family Nurse Practitioner
    • Adultโ€‘Gerontology Primary Care Nurse Practitioner
    • Adult Nurse Practitioner
    • Gerontological Nurse Practitioner
  • Minimum 3-5 years experience as an NP
  • Minimum 1-2 years experience as a clinical leader
  • Active New York State Nurse Practitioner licensure in good standing or must obtain prior to start date
  • Applied for DEA registration prior to hire/start date
  • Must meet all applicable employer and payer credentialing requirements
  • Ability to meet postโ€‘offer requirements, including drug screen and health assessment
  • Compliance with all New York State health requirements
