Change Makers LLC is actively seeking a skilled Licensed Clinical Social Worker (LCSW) to join our dedicated team on a full-time basis. • Provide direct clinical support to clients within the community, focusing on precise assessments and intervention strategies tailored to individual needs.
• Conduct brief, evidence-based screenings and arrange for comprehensive evaluations as required, ensuring timely and appropriate assessments.
• Develop and implement integrated care plans that encompass preventive, treatment, and recovery services, fostering ongoing communication among clients, family members, and other healthcare providers.
• Crisis intervention assistance, responding to immediate crises and determining appropriate next steps for client safety.
• Maintain accurate client records, ensuring the completion of reports and data submission in compliance with organizational standards and regulatory requirements.
• Carry and manage a caseload that includes conducting individual, group, and family therapy sessions focused on achieving client-specific outcomes.
• Participate in interdisciplinary team meetings and committees to enhance service delivery and drive agency-wide quality improvement initiatives.
• Perform on-call duties during and after program hours as assigned, demonstrating flexibility and commitment to client well-being.

As a Licensed Clinical Social Worker at Change Makers LLC, you will primarily serve clients grappling with the complexities associated with major mental illnesses and severe emotional disturbances. Our clients often come from diverse cultural backgrounds, presenting with unique challenges that require a tailored, respectful approach to care.

• A Master's degree in Social Work from an accredited institution, along with current LCSW licensure.
• Expert knowledge in assessment, diagnosis, treatment for mental health issues, and crisis management.
• Strong verbal and written communication skills, with a demonstrated ability to convey complex concepts clearly to clients and team members.
• Effective organizational and planning skills to manage multiple clients and deadlines.
• Capacity to understand and address age-related or specialty issues relevant to the populations served.

• At least 2 years of clinical experience working in a mental health setting, with preference given to those who have experience in underserved communities.
• Familiarity with community health resources and a network of support services to facilitate comprehensive care.
