Position Overview:

Living Water Clinic-Clinica Agua Viva is seeking a dedicated and patient-focused Primary Care Physician with training in Family Medicine (FM) or Internal Medicine (IM) to join our team. The ideal candidate will provide comprehensive healthcare services, build lasting relationships with patients, and develop personalized care plans based on medical history and current health needs.

As a Primary Care Physician, you will play a pivotal role in delivering high-quality, patient-centered care to an underserved community while having the opportunity for professional growth and competitive earning potential.

Responsibilities:
  • Provide medical leadership and collaborate with a multidisciplinary team to ensure integrative patient care.
  • Diagnose, treat, and manage acute and chronic conditions for a diverse patient population across all ages.
  • Offer direct patient care at the clinic and outreach locations within the Tulare County service area.
  • Supervise and support physician assistants and nurse practitioners in clinical practice.
  • Coordinate continuity of care for hospitalized patients and those transitioning from medical respite care.
  • Assist in developing and optimizing electronic health record (EHR) templates and the patient portal.
  • Participate in quality improvement initiatives, ensuring compliance with clinical standards and best practices.

Requirements:
