• Conduct thorough psychosocial assessments to evaluate the emotional, social, and financial needs of patients, ensuring comprehensive care that encompasses all aspects of their well-being.
• Provide both individual and group counseling, addressing critical issues such as grief, adjustment to illness, and coping strategies for patients and their families.
• Work collaboratively with medical teams, including physicians, nurses, and other health professionals, to develop and implement personalized care plans that optimize patient outcomes.
• Manage crisis intervention effectively, providing immediate support to patients and families during difficult situations.
• Coordinate with local community organizations and resources to facilitate post-discharge services and ensure continuity of care, vital for our patients’ ongoing support.
• Participate actively in multidisciplinary team meetings, where your expertise will guide discussions related to patients’ psychosocial needs and inform care decisions.
• Maintain precise and timely documentation compliant with legal standards and hospital policies, which plays a key role in our accountability and transparency.

Our primary focus is on patients receiving hospice care, whom we support during the end-of-life phase. This sensitive population requires compassion, understanding, and specialized knowledge to navigate complex emotional landscapes, adjust to changes, and find solace and peace through the process. In addition, their families and loved ones receive our attention and support to help them cope with grief and provide a nurturing environment.

• A Master's degree in Social Work (MSW) from an accredited program is required.
• Active state licensure as a Licensed Certified Social Worker (LCSW) is essential.
• Strong understanding of social work principles and practices, particularly in hospice or end-of-life care contexts.
• Exceptional communication, interpersonal, and organizational skills to interact effectively with diverse patient populations and interdisciplinary teams.

• A minimum of two years of social work experience in a healthcare or hospital setting is preferred, especially in hospice or palliative care.
• Familiarity with medical terminology, mental health disorders, and crisis intervention techniques will be advantageous.
