Wellsprings of Gilbert, a premier short-term rehabilitation and skilled nursing facility in Gilbert, Arizona, is seeking a compassionate and motivated Occupational Therapist to join our outstanding in-house therapy team. We have Full-Time, Part-Time, and PRN opportunities available for clinicians who are passionate about helping patients regain independence and return home safely.

Qualifications
  • Graduate of an accredited Occupational Therapy program
  • Current Occupational Therapist license in Arizona (or ability to obtain)
  • New graduates and experienced therapists encouraged to apply
Essential Job Duties
  • Evaluate patients with cognitive, physical, and ADL deficits and develop individualized treatment plans
  • Utilize standardized assessments and evidence-based interventions
  • Provide skilled occupational therapy services in accordance with physician orders and regulatory requirements
  • Implement therapeutic exercises, activities, ADL retraining, adaptive equipment training, and other treatment interventions
  • Monitor patient progress and modify treatment plans as appropriate
  • Collaborate with physicians, nursing staff, families, and interdisciplinary team members
  • Educate patients, caregivers, and families regarding treatment plans and functional outcomes
  • Complete timely and accurate documentation
  • Prepare comprehensive discharge recommendations and summaries
