Responsibilities 

  • Comply with all state-mandated Physical Therapy Practice Act and other legal mandates. 

  • Evaluate client for therapy needs and provide written orders/treatment plan for services. 

  • Ensure plan of care is consistent with client assessment and needs. 

  • Instruct other personnel and/or family members and caregivers on intervention goals. 

  • Ensure proper visit documentation completion. 

  • Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider. 

  • Participate in discharge planning.. 

  • Provide in-service education for agency personnel, as needed. 

Requirements 

  • Current unrestricted Physical Therapist license in state for which care is being provided 

  • One (1) year experience as physical therapist, preferably in an in home setting 

  • CPR Certification, required 

  • Two-step negative TB skin test or chest x-ray, required. 

  • Level 1 Fingerprint Clearance Card, required 

  • Valid Driver’s License and auto insurance, required 

  • Understands regulatory compliance with state and federal regulations 

  • Adheres to HIPAA and maintains client confidentiality
