Medication Adherence Pharmacist

Medication Adherence Pharmacists work closely with retail pharmacy colleagues, providers, patients and care givers to provide medication therapy management services and improve adherence for GHS patients. Using tools in the electronic health record and third party systems the Medication Adherence Pharmacist will identify, contact, and work with patients to streamline filling, pick up and/or delivery of prescriptions from Grady Health System pharmacies.

Identify patients with poor medication adherence. Contact patients with poor medication adherence. Navigate barriers leading to poor adherence. Implement strategies to improve adherence. Track overall adherence for targeted patient populations (diabetes, biologics, chronic conditions, i.e.). Complete Medication Therapy Management (MTM). Document MTM activities. Complete prior authorizations. Oversee Medication Access Coordinators assigned to complete prior authorizations. Other duties as assigned.

Registered pharmacist, State of Georgia required. MTM certification preferred 1 year community (retail) pharmacy required. PGY-1 residency with ambulatory or community focus, preferred.
