Providence Medical Group is seeking a full-time nurse practitioner to join Providence Connections Palliative Care in Portland, Oregon. Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families. Join an established interdisciplinary team providing a single, coordinated program for management of chronic conditions or life-limiting illness to patients and families throughout the Pacific Northwest.


  • Full-time position, Monday to Friday
  • Outpatient care setting, community-based settings, and home visits and occasional hospital setting
  • Collaborate with physicians, nurse care manages, social workers, pharmacists, behaviorists, chaplains, and other team members
  • Minimum 1 year of post-grad clinical experience in palliative care, hospice, geriatrics, and/or oncology preferred

Providence Connections is a Palliative Care consultation service for patients living with a serious illness. This interdisciplinary group helps patients and their families find needed resources to control symptoms associated with their illness and maximize quality of life, all while providing compassionate support. Teams of physicians, nurse practitioners, Palliative Care nurses, clinical social workers and chaplains work collaboratively with all members of patients’ care teams to provide holistic care.
