Job Description:

JOB DESCRIPTION

TITLE: Advanced Care Provider ("ACP")

DEPARTMENT: Elliot Professional Services Network, Inc. ("EPS")

REPORTS TO: Chief Medical Officer, Elliot Hospital & EMG Acute Care Services; Sr. Vice President, Chief Operating Officer; Medical Director, Elliot Emergency Medicine Specialists and/or his/her designees

PRIMARY PURPOSE

To provide comprehensive assessment, treatment and education to Emergency Medicine patients. Provides care in a professional, efficient and ethical manner, as well as in accordance with the standards and recommendations consistent with the requirements of all applicable accreditation entities (DNV, Joint Commission, etc., as applicable), applicable laws and regulations of the State of New Hampshire, and all policies, rules and requirements of EPS and Elliot Health System ("EHS").

DUTIES AND RESPONSIBILITIES

Under the direction of the Medical Director, Elliot Emergency Medicine Specialists ("Medical Director"), Sr. Vice President, Chief Operating Officer and Chief Medical Officer, Elliot Hospital & EMG Acute Care Services and/or his/her designees and within established EHS and departmental policies and procedures, ACP shall perform the functions set forth below. All duties shall be carried out: (i) in accordance with all applicable laws, rules and regulations, including those concerning protection of patient confidentiality; (ii) in a professional and competent manner and in accordance with the generally accepted standards of care; and (iii) in accordance with all policies, rules and requirements of EPS and/or EHS, and all bylaws, rules and regulations and policies of the medical staff of Elliot Hospital, as they may be amended from time to time.

Essential Functions:

1. Patient Assessment: Provide assessment and examination of individuals by interviewing the patient and/or family members to obtain an appropriate history, with regard to the patient's physical, developmental status and behavior. Examines the patient to obtain objective physical data, performs appropriate systems review as indicated by the evaluation. Completes a physical assessment based on interview, examination and available data. Identifies medical and health issues and needs based upon the evaluation.

2. Treatment Planning: Based on the patient evaluation, makes appropriate treatment plans. ACP shall at all times serve the best interests of the patient and shall exercise his or her best independent professional judgment when rendering care. Develops and implements a treatment plan, which may include diagnostic tests, pharmaceutical and non-pharmaceutical interventions, medical and surgical services as well as education. This may be part of a multidisciplinary team. Reviews clinical data from lab results, x-rays, scans and pathology reports.

3. Coverage Requirements: The Medical Director and Chief Medical Officer, Elliot Hospital & EMG Acute Care Services and/or his/her designee will annually determine necessary hours of coverage for agreed upon locations. ACP will collaborate with the Medical Director to determine coverage schedule. Ensures that any time out of the practice is provided in advance and ensures that practice coverage is arranged prior to departure.

4. Documentation of Services: Maintains ongoing documentation of patient interventions. All documentation must be completed promptly but no later than forty-eight (48) hours after the date of service. This includes documentation of initial assessment, treatment plans, interventional and ancillary services and any other patient interactions. Documentation to be completed using an Electronic Medical Record ("EMR") as provided by EHS. Required to cooperate with any audits of patient charts conducted to ensure compliance with all coding and documentation requirements and any compliance education reasonably required by the Medical Director and Chief Medical Officer, Elliot Hospital & EMG Acute Care Services and/or his/her designee.

5. Participates in Quality Improvement Activities: ACP will be required to attend and provide input in quality improvement and quality assurance projects as deemed necessary by the Medical Director and Chief Medical Officer, Elliot Hospital & EMG Acute Care Services and/or his/her designee. Such quality improvement activities will include activities related to enhancing and maintaining patient satisfaction. Participates in quality management activities that at a minimum comply with all applicable accreditation entities (DNV, Joint Commission, etc., as applicable), and the bylaws of the Medical Staff of the Elliot Hospital.

6. Patient Satisfaction: Demonstrates courtesy, respect and caring to patients and families. Surveys will be distributed to patients at the close of their visit. It is expected that the resulting information will be utilized to ensure that ACP is within usual bounds when compared to colleagues within the specialty.

7. Participates in Practice Functions/Billing: ACP will be required to work with staff to maintain the efficient operation of the practice and collaborate with nursing, support personnel, and administration in matters affecting efficient operations. ACP will adhere to policies, procedures, standards of care, and standards of infection control applicable, as revised from time to time. Participates in staff meetings and cooperates with practitioners and co-workers to achieve departmental objectives. Reports concerns and problems as they arise to supervisor. Demonstrates courtesy, respect and caring to physicians and coworkers and maintains professional rapport with external parties and organizations. Participates in staff meetings.

8. Collaboration: Collaborates with physicians, nursing, support personnel and administration in matters affecting patient care. Participates in crafting policies, procedures, standards of care, standards of practice, standards of infection control, etc. Follows the processes and practices of EHS designed to meet the specific needs of the patients, e.g., access needs, educational needs, age-specific needs, medical surveillance, etc. Ensures the protection of patient rights and adherence to HIPAA regulations.

9. Education: Participates in planning, developing, implementation and evaluation of education programs, support and services offered by the EHS, which includes attendance at a reasonable number of corporate and Hospital meetings and education opportunities. EHS is continually expanding services and capabilities and is increasing its use of an EMR, which episodically requires a significant time commitment by providers to develop proficiency and familiarity. As such, attendance and participation in educational and training programs or sessions will be compensated, as determined by Medical Director and Chief Medical Officer, Elliot Hospital & EMG Acute Care Services and/or his/her designee at administrative hourly rate.

10. Scheduling: Provides patient contact hours as scheduled by the Medical Director in the Emergency Department and Urgent Care Locations. This time will be managed by ACP to ensure all patient obligations are met, participation in various committees and practice meetings and coverage of a colleague for vacation, etc. ACP shall be responsible for arranging clinical coverage by another Elliot Emergency Medicine Services Advanced Care Provider for all unplanned absences. Deviation from the required hours requires approval by the Medical Director. Full-time advanced care providers are required to work a minimum of one thousand, one hundred sixteen (1,612) hours each year. This annual hourly requirement must be achieved by working four hundred three (403) hours per quarter.

11. Credentialing: Submits credentialing and re-credentialing information to Medical Staff Services within 20 days of receipt of application.

12. Location of Services: Services will be provided at the assigned locations as assigned by the Medical Director and Chief Medical Officer, Elliot Hospital & EMG Acute Care Services and/or his/her designee.

13. Policies and Procedures: Adheres to all policies and procedures including but not limited to; infection control procedures, safety procedures, universal precautions, patient confidentiality and security policies. Ensures confidentiality of employee, legal, client/patient, budget and all company matters.

14. Space and Support Staff: Employer will provide ACP with appropriate and adequate support staff, space, and materials to meet his or her obligations under this Agreement.
