Depending on licensure and role, responsibilities may include:
  • Evaluate patients and develop individualized occupational therapy plans of care.
  • Provide skilled occupational therapy services in the patient's home.
  • Help patients improve independence with activities of daily living, mobility, safety, and functional performance.
  • Provide therapeutic exercises, functional activities, strengthening, balance, coordination, and adaptive equipment training.
  • Assess and address home safety concerns and recommend appropriate modifications or adaptive equipment.
  • Educate patients, caregivers, and family members regarding home exercise programs, safety, fall prevention, and strategies to maximize independence.
  • Monitor patient progress and modify interventions as clinically appropriate.
  • For COTAs, implement established plans of care under the direction and supervision of a licensed Occupational Therapist.
  • Communicate patient progress, concerns, and changes in condition with the supervising OT and interdisciplinary care team.
  • Coordinate care with physicians, nurses, physical therapists, speech therapists, social workers, home health aides, and other members of the care team.
  • Complete accurate, thorough, and timely clinical documentation.
  • Participate in care coordination and discharge planning.
  • Maintain compliance with applicable home health, Medicare, state, and agency requirements.
Qualifications
Occupational Therapist (OT)
  • Current Virginia Occupational Therapist license in good standing.

  • Strong communication, documentation, and patient-care skills.
Preferred for Both Roles
  • Previous home health experience is preferred.

  • Familiarity with electronic medical records (EMR/EHR).
  • Reliable transportation and the ability to travel throughout the assigned service area.
  • Compassionate, dependable, patient-centered approach to care.
What We Offer
  • Competitive compensation based on role and experience.
  • Full-time, part-time, and PRN opportunities.
  • Flexible scheduling and greater autonomy in managing your workday.
  • Supportive clinical and administrative leadership.
  • Collaborative interdisciplinary team environment.
  • Opportunities to grow with a rapidly expanding home health agency.
  • Training and support for clinicians who are newer to home health.
  • Meaningful one-on-one patient care without the constant pace of a facility-based setting.
  • The opportunity to help patients remain safe, functional, and independent in their own homes.
Why Home Health?
