We’re building a team of dedicated clinicians who are already credentialed with insurance providers, experienced in delivering high-quality care, and driven by a genuine heart to serve and heal. Every interaction with our members is designed to be tailored to their unique needs, preferences, and lifestyle — so they feel seen, heard, and supported at every step of their health journey.

As we prepare to launch this fall, we’re looking for Primary Care Providers who want to be part of something new, meaningful, and member-centered. This is a per diem role to start, with the potential to grow into part-time or full-time based on practice growth and membership volume.

Role Overview:
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive medical care to patients aged 18 and up, focusing on preventive care, diagnosis, and treatment of various health conditions. This role requires a strong commitment to preventive-first care and the ability to work collaboratively within a multidisciplinary team.

Key Responsibilities:
  • Provide in-person, virtual, or in-home primary care visits (depending on membership plan).
  • Conduct routine physical exams, screenings, and wellness assessments for adult members.
  • Develop and manage individualized care plans in collaboration with our care team.
  • Educate members on preventive health strategies and chronic disease management.
  • Coordinate referrals and collaborate with specialists when needed.
  • Maintain accurate, up-to-date patient records and documentation and management.
  • Provide education to patients regarding health maintenance and disease prevention.
  • Participate in regular team check-ins and contribute to continuous improvement of the member experience.

Requirements:
  • Active license in D.C., Maryland, or Virginia as one of the following:
    • Family Nurse Practitioner (FNP)
    • Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
  • Current credentialing with major insurance providers strongly preferred.
  • Minimum of 2 years of direct adult primary care experience preferred.
  • Must have professional liability insurance or willingness to obtain it.
  • Excellent bedside manner with a genuine desire to serve, heal, and personalize care.
  • Ability to work independently and collaboratively with a small, agile team.
  • Strong communication and organizational skills.
