Geriatrician Opportunity

Sutter Medical Group of the Redwoods (SMGR) is recruiting a Board-Certified Geriatrician to join our established Geriatric practice within Sutter Senior Care.

This is an exciting opportunity to practice in a team-based, value-driven outpatient setting focused on delivering high-quality care to older adults, particularly those with complex medical and social needs.

The ideal candidate will have experience in a geriatric group practice or multi-specialty outpatient setting, with a strong background in primary care geriatrics and interdisciplinary team-based care.

Key Responsibilities:

  • Provide comprehensive outpatient care for geriatric patients
  • Diagnose and manage chronic and complex conditions (e.g., diabetes, hypertension, heart disease, cognitive disorders)
  • Deliver preventive care, screenings, and immunizations tailored to seniors
  • Manage polypharmacy and medication safety
  • Collaborate with APCs, nurses, social workers, and specialists to deliver coordinated care
  • Coordinate referrals and support care transitions across settings
  • Educate patients and caregivers on disease management and healthy aging
  • Participate in team huddles, quality initiatives, and value-based care programs

Qualifications:

  • MD or DO; Board Certified in Geriatrics, Internal Medicine, or Family Medicine
  • Valid and unrestricted California medical license (or eligibility)
  • Experience in outpatient geriatrics or primary care preferred
  • Experience in interdisciplinary, team-based care models preferred
  • Strong clinical, communication, and patient engagement skills
  • Passion for caring for older adults and underserved populations
