Each associate provides a service that directly or indirectly affects patients, families, other team members, and the public. It is our goal to DELIVER excellent and patient experiences to every patient and resident family member.

Hospice CNA 

Shift Available: PRN

Service Area for Coverage: North, Northwest, South, Southeast

Responsibilities:

  • Provide exceptional care in coordination with the assigned nursing care team.
  • Maintain patient records that include any changes in doctorโ€™s orders, diet, or personal living activities.
  • Help with patient meals, feeding, and basic activities of daily living.
  • Deliver weekly reports to the case manager and review any important changes in patient conditions.
  • Educate patients and family members.
  • Prepare timely, complete, and accurate clinical notes.
  • Communicate effectively with healthcare providers and other care team members.

Qualifications:

  • Must live in Central Houston (Inner Loop)
  • CNA license in the state of Texas (required).
  • One (1) year of hospice experience.
  • Able to lift up to fifty (50) pounds.
  • Able to work occasional weekends, holidays, evenings.
  • Strong communication and people skills.
  • Experience with HCHB EMR (preferred but not required)
