Job description

The primary role of a Care Partner at Cottages of Lake St Louis is to protect, sustain and nurture the up to 10 elders who live in their Cottage. This is done by providing assistance with their activities of daily living and meeting other needs, as they are required. The ultimate goal of the Care Partner is to provide assistance and care which promotes a high quality of life and a positive elderhood. The responsibilities include not just care for the Elders, but also their environment, including cooking, laundry, and some housekeeping.
Functions of the job:

  • Recognize/respond to the needs of the elders while encouraging their independence
  • Observe, monitor, and report changes in the elders' condition to nurses on the Clinical Support Team
  • Assist elders with personal care functions, including but not limited to:
  • Dental and mouth care
  • Bathing, dressing, and grooming
  • Toileting and incontinence care
  • Assist in moving, repositioning, and transporting elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. as specific to each elders plan of care
  • Attend and participate in elder care conferences
  • Be a part of a team of care givers in the home who work collaboratively to provide care in a healthy and safe environment
  • Prepare and serve meals and nutritional snacks
  • Assist elders with eating and properly document the intake and weights of the elders
  • Provide care to the elders in a gentle, empathetic, efficient, and timely manner
  • Launder clothing, linen, and other articles per guidelins
  • Routinely clean and maintain the environment of the home
  • Relay grievances or suggestions to the Guide

Each Care Partner works with the other Care Partners in their Cottage as part of a Self Managed Work Team (SMWT). The SMWT works collaboratively with the nurses who are part of the Clinical Support Team to direct all matters related to skilled nursing services. For all other matters the Care Partner and SMWT report directly to the Guide of their Cottage.

  1. Certified Nurse Assistant certification
  2. High school graduate or equivalent
  3. Twist, stoop, bend, squat, lift, and carry items up to 25 pounds
  4. Reach above, at, or below shoulder height
  5. Walk or stand a majority of the time
  6. Communicate orally and/or in writing with elders and coworkers
  7. Ability to visually monitor elders from a distance of up to 35 feet, and to hear elder's calls, coughs, and alarms
