For close to two decades, Health Link Home Health and Hospice has been committed to providing exceptional healthcare services to patients in the San Francisco Bay Area, all in the comfort of their homes. As we continue to expand our reach, we are looking for a skilled Account Manager for Contra Costa County to join our team and advocate for our home health and hospice services.

At Health Link Home Health and Hospice, your primary responsibility will be to help leverage Health Link's extensive relationships in the community to build and reinforce trust in our home health and hospice programs.


Home Health is about trust - in order for nurses, social workers, and doctors to trust us with their patients - we must keep our promises and live out our values. Your responsibility is to assist the front line of our home health team and give more patients the opportunity to be cared for by our amazing clinicians.


You will work with our Marketing Team and Director of Community Relations to execute a sales and marketing plan and achieve our goals. To be successful in this role, you will need a keen grasp of sales and marketing and a concept of the home health and hospice industry is a plus. Regardless if you have experience in home health or not you will need to digest information rapidly and learn as much as possible as fast as possible.

Responsibilities

  • Assist in implementing a comprehensive marketing / sales plan for Health Link Home Health agency.
  • Develop and maintain new and current relationships with community partners including hospital CM / SW teams, doctor office and clinic teams, senior living communities, and skilled nursing facilities.
  • Ride alongs with current team members, respecting existing relationships, and developing your own.
  • Calls to schedule meetings, in-person visits to present our services, and a TON of follow up.
  • Collaborate with the intake team to coordinate referrals and be the liaison between our community partners and internal staff. Lots of messaging and coordination calls.
  • Represent Health Link at relevant community events and meetings to build awareness, develop relationships and build our network.
  • Manage your own budget, including tracking expenses and ensuring effective use of resources.
  • Identify, target, and develop new relationships into reliable referral partners.
  • Always have your phone, always respond, unless on a planned vacation.
  • Maintain impeccable calendar discipline.
  • Maintain impeccable CRM logging discipline.
