You will be responsible for assessing and addressing the complex needs of patients, helping them regain their independence and improve their quality of life.

About the Organization

At Alpha Behavioral Wellness Therapy, we are deeply committed to empowering individuals recovering from injury, illness, or disability. Our comprehensive rehabilitation center specializes in delivering exceptional care through an interdisciplinary approach, integrating physical therapy, occupational therapy, and speech-language pathology. Our expert staff employs evidence-based practices and the latest rehabilitation technologies to ensure our patients receive personalized and effective treatment tailored to their specific needs.

Work Environment

This role is designed to be remote, allowing for flexibility while supporting our patients at the hospice center. Our environment fosters collaboration among healthcare professionals. You will frequently engage with a multi-disciplinary team, including physicians, nurses, and rehabilitation specialists, ensuring a holistic approach to each patient's care. Although the position mainly involves telehealth services, there may be occasions when direct patient interaction in hospice settings is necessary to assess needs and support families.

Key Responsibilities
  • Conduct thorough psychosocial assessments through interviews, observations, and a review of medical records, evaluating the emotional, social, and environmental factors affecting each patient's recovery.
  • Provide individualized counseling and therapeutic support, helping patients and their families navigate adjustment issues, emotional challenges, and coping mechanisms throughout their rehabilitation process.
  • Work closely with the rehabilitation team to develop and implement personalized treatment plans that address the psychosocial needs of patients, ensuring comprehensive support.
  • Assist patients in identifying and accessing critical community resources, such as support groups, vocational rehabilitation services, and housing assistance, to facilitate their recovery and reintegration into the community.
  • Educate patients and families about available community resources and post-rehabilitation services, empowering them to make informed decisions about their care.
  • Engage in discharge planning, working to ensure smooth transitions for patients moving to home care or other healthcare settings.
Patient Population / Client Focus

You will primarily work with patients at various stages of recovery from acute medical conditions or disabilities, including those with chronic illnesses, neurological disorders, and those undergoing rehabilitation post-surgery. Understanding the unique emotional and social challenges faced by these individuals is crucial to providing effective support and promoting their overall well-being.

Qualifications
  • Master's degree in Social Work from an accredited program, demonstrating a solid foundation in clinical practice.
  • Valid and active state licensure as a Licensed Clinical Social Worker (LCSW), ensuring compliance with regulatory standards.
  • Exceptional communication and interpersonal skills, allowing you to build rapport and trust with patients and families.
Preferred Experience
  • Prior experience in a rehabilitation or medical setting, enhancing your understanding of patient-centered care and the rehabilitation process.
  • A strong grasp of issues related to adjustment, grief, and various coping strategies, which is essential in facilitating patient resilience.
Compensation and Benefits

The salary for this position ranges from USD 75,000 to 100,000 annually, alongside a competitive benefits package that includes comprehensive health insurance coverage for employees and their families, retirement plans with employer contributions to ensure your financial security, and generous paid time off. Additional perks include professional development opportunities through conferences and workshops, designed to enhance your skills and foster career growth.
