Job Title: Lifesharing Program Specialist
Location: Lehighton, Pennsylvania (Community-Based & In-Home Visits)
Employment Type: Full-time

About the Role
We’re hiring a Lifesharing Program Specialist to support and oversee in-home Lifesharing arrangements across Pennsylvania. In this role, you’ll partner with providers who open their homes to adults with intellectual and developmental disabilities. You’ll ensure quality, compliance, and meaningful outcomes—serving as a trusted resource for families and individuals alike. If you're passionate about person-centered services and relationship-based care, this is the role for you.

Responsibilities

●        Conduct regular home visits to monitor the health, safety, and well-being of Lifesharing participants

●        Collaborate with providers to ensure compliance with ISP goals and regulatory requirements

●        Provide training, coaching, and resources to support Lifesharing families

●        Facilitate interdisciplinary team meetings and develop support strategies

●        Complete documentation, assessments, and reports in a timely manner

Required Qualifications

●        Must meet one of the following education and experience combinations:
• Master’s degree and 1 year experience in human services

• Bachelor’s degree and 2 years of experience
• Associate’s degree/60 credit hours and 4 years of experience
• High School diploma and 6 years of experience

●        Valid Pennsylvania driver’s license and reliable transportation

●        Ability to pass PA Criminal, Child Abuse, and FBI clearances

●        Strong communication, organizational, and interpersonal skills

Preferred Qualifications

●        Experience with intellectual/developmental disabilities (IDD) services

●        Prior work with Lifesharing or foster-style models

●        Background in case management, residential services, or behavioral health
