Job Summary and Qualifications

ยท Demonstrates advanced clinical skills and ability to assess patients to determine appropriate level of care.

ยท Identifies cultural considerations, gender identity and preferred language for patients and family

ยท Demonstrates advance practice in assessments, and all other documentation timely and in compliance with requirements (as appropriate to scope)

ยท Assesses environment and patientโ€™s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition

ยท Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)

ยท Recognizes and acts upon unexpected assessment findings

ยท Leads in ensuring a safe structured milieu with evidence based treatment modalities/resources

ยท Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers

ยท Provides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.

ยท Assesses, collaborates, coordinates and provides guidance on complex discharge planning needs and readiness for discharge with the multidisciplinary team.

ยท Develops and maintains a community resource guide including access to public assistance/alternatives.

ยท Facilitates and participates in Community Education, Activities and Support Groups (as appropriate to scope)

What qualifications you will need:

ยท Basic Cardiac Life Support must be obtained within 30 days of employment start date

ยท Masters Degree - REQUIRED

ยท Advanced Licensure - REQUIRED โ€“ (LCSW) Clinical Social Worker, or (LPC/LMHC) Licensed Professional/Mental Health Counselor, or (LMFT) Licensed Marriage and Family Therapist
