Eminence Home Care is seeking a dedicated Intake Coordinator to join our team in Rocky Hill, CT. In this essential role, you will be the first point of contact for prospective clients and their families, guiding them through the intake process and ensuring that they receive the support and services they need. You will collaborate with various departments to facilitate a smooth transition for new clients into our care programs.

Responsibilities:

  • Conduct initial assessments and gather necessary information from prospective clients and their families.
  • Assist clients through the intake process, ensuring all documentation is completed accurately and efficiently.
  • Collaborate with case managers to determine appropriate services and establish care plans.
  • Maintain accurate records and follow up on referrals and inquiries.
  • Provide ongoing support and information to clients and their families to address questions and concerns.
