Position Summary

The Program Coordinator, Children's Heart Institute coordinates, implements, and supports the Children's Heart Institute Patient Navigation Program while leading patient-centered programs and strategic initiatives that advance the goals of the Children's Heart Institute. Under the direction of the Service Line Director, this position serves as a key programmatic and operational partner responsible for supporting patient access strategies, facilitating multidisciplinary collaboration, coordinating service line initiatives, and leading projects that enhance patient and family experience, quality outcomes, continuity of care, and service line growth.

Essential Functions and Responsibilities of the Job

  • Functions as a central resource for physicians, clinical teams, patients, families, referring providers, and community partners by facilitating communication across the continuum of care, coordinating patient-centered programs, supporting program development activities, and implementing initiatives that improve patient access, continuity of care, and organizational effectiveness.

  • Supports the development, implementation, and evaluation of Children's Heart Institute programs and strategic initiatives through project management, non-clinical patient navigation activities, performance monitoring, operational coordination, and cross-functional collaboration.

  • Exercises independent judgment in managing multiple priorities while supporting organizational goals related to patient experience, quality improvement, physician engagement, service line growth, and operational excellence.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Requirements

Qualifications/Work Experience:

  • Minimum 3 years' experience coordinating programs, patient navigation services, care coordination activities, or service line operations in a healthcare setting preferred.

  • Experience working collaboratively with physicians, multidisciplinary teams, patients, families, and community partners preferred.

  • Proficient in Microsoft Office 365 and experience developing reports, presentations, or performance-tracking tools preferred.

Education/Licensure/Certification:

  • Bachelor's degree in healthcare administration, business administration, public health, organizational leadership, communication, project management, or related field required.
