• Conduct comprehensive psychosocial assessments of seniors, identifying their emotional, mental, and social needs.
• Develop individualized care plans in collaboration with clients and their families, ensuring that each plan addresses their specific goals, preferences, and concerns.
• Provide counseling and emotional support to seniors dealing with various life transitions, including health crises, bereavement, or significant lifestyle changes.
• Act as an advocate for clients, helping them navigate community resources and ensuring they have access to the necessary services and supports.
• Collaborate with interdisciplinary teams to create a seamless service provision, including healthcare professionals, caregivers, and family members.
• Maintain regular contact with clients through visits—whether virtual or in-person—to monitor their well-being and adjust care plans as needed.
• Educate seniors and their families about community resources, support groups, and enhancement programs that foster social engagement and well-being.
• Keep detailed and accurate documentation of client interactions, assessments, and care plans in accordance with ethical and legal standards.
• Participate in ongoing professional development to stay updated with social work practices, policies, and the latest community resources.

This role specifically focuses on seniors within our community, addressing their unique physical, emotional, and social challenges. The LCSW will work with clients ranging from those requiring minimal support to those facing significant life changes or health issues, tailoring interventions to meet the diverse needs of this population.

• Master's degree in Social Work from an accredited institution.
• Current state licensure as a Clinical Social Worker (LCSW) is essential.
• Proven ability to empathize with and advocate for senior clients and their families.
• Strong communication skills for effectively interacting with clients, families, and interdisciplinary teams.
• Proficiency in electronic health record (EHR) systems and relevant software applications.

• A minimum of two years of experience working in a similar social work role, ideally within a community center or geriatric care setting.
• Experience with interdisciplinary collaboration and case management.
• Familiarity with aging-related issues and local community resources.
