About the Opportunity

Stepping Stone Behavioral Health is seeking dedicated and compassionateLicensed Social Workers (LSW/LISW), Licensed Professional Counselors (LPC/LPCC), or Licensed Chemical Dependency Counselors (LCDC II, LCDC III, LICDC)to join ouroutpatient behavioral health team.
Thismobile clinician roleprovides mental health and substance use counselingdirectly inside skilled nursing and long-term care facilities, offering meaningful support to residents coping with emotional, behavioral, and substance-related challenges.

What You'll Do

  • Conductbio-psychosocial assessmentsand develop individualized treatment plans.
  • Provideindividual and group counseling, teaching coping and recovery skills.
  • Identify resident needs, set measurable goals, and guide them toward emotional stability.
  • Participate intreatment team meetingsand collaborate with facility staff and community partners.
  • Maintain timely and accurateclinical documentationin EMR systems.
  • Assist withtelehealth sessionsand submit required daily reports.
  • Facilitate safe discharge planning and coordinate aftercare services.
  • Uphold confidentiality, professionalism, and compliance with facility and state regulations.

Requirements

  • Must be21 years or olderwith avalid driver's license.
  • ActiveOhio license: LSW, LISW, LPC, LPCC, LCDC II, LCDC III, or LICDC.
  • Ohio Medicaid Billing NumberandNPI Numberrequired (or ability to obtain).
  • Minimum of anAssociate's Degree(Bachelor's or Master's preferred).
